Financing Eye Care at CVP Physicians Dayton
When you visit a CVP Physicians Dayton clinic, bring all insurance information, including insurance cards, and a form of payment. For your convenience, we accept Visa, MasterCard, Discover, and American Express.
Payment, including co-payments, are expected at the time of service. We are happy to file your insurance claim for you if you provide the appropriate insurance information and any required referrals before your appointment.
Many medical insurance companies, as well as HMOs, require pre-authorization for medical services. Please consult with your insurance company or primary care physician to obtain a pre-authorization for services, if required, prior to the date of your appointment at CVP Physicians Dayton.

Please Note:
This list is not all-inclusive. Please verify your coverage prior to your visit by calling the number on the back of your insurance card.
You can also use your Health Savings Account (HSA) or Flexible Spending Account (FSA) to pay for eye care that is not covered by insurance with pre-tax funds. Check with your plan administrator to learn if your plan can be used for procedures like LASIK or to cover out-of-pocket costs for other eye care needs.
Late Arrival, Cancellation & No-Show Policy
The following guidelines help us provide timely and quality care to all patients.
Late Arrival
- Patients are considered late if they have not arrived within 10 minutes of their scheduled appointment time.
- Late arrivals may result in rescheduling or a reduced service depending on availability.
Cancellation
- Appointments must be canceled or rescheduled at least 24 hours in advance.
- Late cancellations may result in a $35 fee.
Missed Appointment
- Missing an appointment without prior notice is considered a no-show.
- No-shows may be charged a $35 fee.
- If missed appointments persist this may affect future scheduling.
Compassionate Exceptions
- We understand that life can be unpredictable.

At no time will the minimum payment due be less than $27.00. Minimum interest charge is $3.00 per credit plan. Standard variable APR of 32.99%, based on the Prime Rate.
